Trabeculectomy

Trabeculectomy has been the "gold standard" glaucoma surgery for many decades. It creates a new drainage channel that allows fluid to flow from inside your eye to a small, controlled reservoir (called a "bleb") under the thin membrane covering the white of your eye.

How Trabeculectomy Works

Think of trabeculectomy as creating a new "drain" for your eye when the natural one is clogged. The procedure involves:

  1. Making a small flap in the white part of the eye (sclera)
  2. Creating a new opening under this flap for fluid to drain out
  3. Using special stitches to control how much fluid flows through the new drain
  4. Creating a space under the thin membrane (conjunctiva) where the drained fluid collects before being absorbed by the body

Tube Shunt Surgery (Paul, Baerveldt, and Ahmed Tubes)

A tube shunt, such as the Paul, Baerveldt, or Ahmed tube, is a small device implanted in your eye to create a new drainage pathway. It consists of a tiny tube connected to a plate. The tube is placed into the front chamber of your eye, and the plate is positioned under the thin membrane covering the white of your eye.

How Tube Shunt Surgery Works

  1. Placing the plate portion of the implant on the surface of the eye, underneath the thin membrane (conjunctiva)
  2. Inserting the small tube into the front chamber of the eye
  3. Covering the exposed part of the tube with tissue to protect it
  4. The tube allows fluid to flow to the plate, where it's absorbed by surrounding tissues
